ADMISSION PROCEDURE :

Pre-Admission Clinic:

Kindly visit our pre-admission clinic to ensure that the contractual requirements of you Medical Aid Fund are met. Your admission and other administration issues will be discussed with you. You can also phone us at: 016 454 6000 and ask for the Pre –Admission Clinic.

On admission to Midvaal Private Hospital, the following will be required of you.

• You will be requested to complete an ‘Admission Form’ and will also verify the person will be responsible for your account.
• Depending on the terms and conditions of you’re Medical Aid the member will be required to make a co-payment. The amount usually depends on your Medical Aid and the plan you are on.
• For registration, you will be asked for:
• a copy of your ID and Medical Aid Card,
• Authorization number, Procedure and ICD 10 codes from you treating doctor. No patient can or will be admitted to hospital without his/her authorization number from their Medical Aid
• If you don’t have a copy, a member of staff will copy this for you. In order to verify that the copies are valid, you will be asked to sign the copies.
• You will now be escorted to your room by one of the ward staff, who will also transport your luggage for you.
• Once you arrive, a nurse or coordinator will explain the   facilities in your room and you will be asked to choose your meals.
• The nurse will ask you to change your clothes into pajamas. Your temperature and blood pressure will be checked. The necessary questions regarding you health will be asked
• If you need to undergo any – minor or major – surgery or a procedure to be done, you will be asked to the sign consent form in order to grant permission for the procedure to be undertaken...
• If you have any questions at this point, please do not hesitate to ask our nurse or request to see a coordinator.

• ID
• Copy of your insurance documents (for insurance claim only) or Medical Aid Card
• Admission Form
• Consent of Medical or Surgical Treatment

• ALL ADMISSION FORMS FOR PRE-BOOKED ADMISSION MUST BE HANDED IN 48 HOURS BEFORE ADMISSION
